Please notify the office of any change in your address or telephone number so the office records are up to date at all times.

Moving??? Want a memory book? If you are checking out of school and contributed to the Middle School Fair, you are entitled to a Memory Book. You will need to give your homeroom teacher $2.00 in postage stamps and your new address.

5.130 Withdrawal and Transfer
The procedure for withdrawal or transferring is as follows:

1. Secure authorization for withdrawal or transfer from your parent or guardian.

2. Obtain appropriate forms from the principals office.

3. Have the forms filled out by teachers, return all school books and property, and make sure all fees are paid.

4. Take completed forms to the principals office for final clearance.
