2.050 Physical Education
Physical education is a required course for all middle-school students unless excused by a physician for medical reasons. Each student must provide his/her own clothing, and all should be clearly marked in permanent ink. Failure to bring proper clothing will cause a studentís grade to be lowered. Clothing should be taken home on weekends and washed. Students are encouraged to shower after classes involving physical activity. Students issued P.E. locks must pay a $10 replacement fee if lost.

2.060 Physicals, Immunizations and Birth Certificates

Physicals:

All students upon initial entry into Alliance City Schools and at the 7th Grade must have a physical examination by a doctor and present a medical report to the school principal.

Conditions:

1. A physical administered by a qualified physician within a six month period shall be accepted as meeting the district requirement.
2. All 7th Grade students are required to have a physical prior to entering grade 7.
3. The cost of the physical shall be borne by the parent or guardian of the student.
4. No student will be allowed to attend school without evidence that the requirements have been fulfilled.

Immunizations:

Nebraska Law requires that each child be immunized as follows: 3 - D.P.T., 3 - Polio, 1 - Measles, 1 - Rubella, and 1 - Mumps. ALL students MUST have received their 2nd MMR before entering school in the fall.  Students entering the 7th grade MUST ALSO have received the complete hepatitis B series.

Each child must have a medial record of the month and year of the required shots.

Conditions:

1. See nurses or office personnel for the proper school immunizations regulation forms. This must be completed before a student can attend school.

Birth Certificates:

The Alliance Public Schools require that each student provide the school principal with an official state birth certificate prior to attendance in the school system.

Conditions:

1. The parent or guardian may instead choose to provide both a hospital birth certificate and a notarized affidavit explaining why there is no certified copy.
